Mediksys is a clinic management system designed to help the clinica terencio and hospital in managing their patient record and accommodating during their check-up. This system has Features include save, delete, edit, refresh, search by either first name, surname or id. The search also includes autocomplete. Crystal report with option to print a single selected record or all the records in a datatable. Also a flexibility in the DataGridView which populates all fields when a record is clicked on the gridview. (Note: this is very simple make it complex but don’t be redundant in explain the part. Scope of the System The authorized nurse (receptionist) and Doctor can access to the system during office hours. The patient can view their Number on the screen for their check-up The authorized nurse (receptionist) can edit, add and delete patient information. The Doctor handle or control the patient Express Assist (PEA) B. Limitation of the system It does not cover other processes aside from recording patient record and viewing the patient number for check-up. Review on Related Literature and Studies According to Anonuevo (2009) the most high-volume medical service facilities, the implementation of an efficient queuing and scheduling system would always pose a problem. More often than not, medical service facilities would implement a queue manually by either assigning a queue administrator or by using a ticketing system. The problem with these implementations is that the manager or facilitator of the queue does not have a convenient and quick way of informing queued customers of any updates. Because of this, patients are given no indication or estimation as to when their turn will be. As such, patients who are waiting in line will be forced to wait in the same vicinity as the facility. Which in turn, would  lead to long and crowded queues in that particular facility. Another problem that most Philippine medical service facilities have is implementing a reliable, robust and efficient record keeping system. In the Philippine setting, most service facilities still choose to use a paper-basedsystem for keeping medical records. This system has already proved to be slow and inefficient. In a recent survey conducted by the group, 63% of its respondents have admitted to having problems with their record keepingsystem. 83% of these respondents have also admitted to misplacing patient medical records or losing them entirely. These are unacceptable numbers, especially for a medical service facility. Provincial Hospital In-Patient System or PROV-HIS is a generic transaction processing system for provincial hospitals. Implemented in Visual Basic 5 Enterprise edition, it is designed to integrate hospital functions in a local area network system. The proponents identified that data management workload is uneven in the existing hospital system. By carefully studying and establishing a paper trail, the proponents managed to evenly distribute and systematize work among hospital personnel hence disburdening some hospital personnel of work backlogs, made possible through PROV-HIS. L. Morco, a master degree student of De La Salle Univesity observed that PROV-HIS answers the need for an efficient hospital information management. It made use of data coming from the vital source of hospital transactions which are the admission, the medical record service, the billing service and the hospital wards. The result of the proponents effort is a system that would serve as benchmark in the coming years particularly on hospital in-patient systems for devolved, tertiary provincial hospitals. Tools such as systems design and analysis, knowledge on database management, normalization and creation, software engineering methodologies, human-computer interaction theories and information systems concepts were benefited by the system developers. The study concludes that the developed system is now utilizing hospital data with great accuracy and  efficiency with the use of non-redundant processes and less error-prone activities compared to the existing system. 1.What does Ishmael say the war is about? Ishmael says nothing about the causes of the war, or what each side was fighting for, or of the overall political and social conditions in Sierra Leone that caused the war. This was a deliberate strategy on the part of Beah, the author. He wanted to present the war through the eyes of a child. As a boy of twelve, when the war first affected him, he had no interest in politics. He had no reason to be interested—his main interest, understandably for a boy of his age, was in singing and dancing to rap music and hanging out with his friends. When the war comes to him, it is for him a battle for personal survival, not a political cause. He is also fueled by feelings of revenge—instilled into him by his army officers—against the rebels because they killed his family. Once again, these are personal feelings not political beliefs. For the reader, then, transported to a land he or she knows nothing about (for the American reader, that is), the war seems not only almost un imaginably brutal but also meaningless. It consists of one side mindlessly killing the other, and vice versa, in skirmishes in small villages. Ishmael does report Lieutenant Jabati’s speeches to his men, in which he says they are defending their country (â€Å"We kill them [the rebels] for the good and betterment of this country† [p. 123]), but such appeals to patriotism are not what inspire Ishmael. Ishmael’s ignorance of politics is again stressed when he is in Freetown during his rehabilitation and sees a convoy of cars and military vans. He is told that the new president, Tejan Kabbah, who had won an election eight months earlier is passing by. â€Å"I had never heard of this man,† Ishmael writes pointedly. This confirms the tenor of the book as a whole: Ishmael is a boy caught up in a war he knows nothing about for a cause he does not care about. 2.Why was the war fought and what course did it take? During the 1980s Sierra Leone was a one-party state governed by the All-People’s Congress (APC) party. However, this period was marked by extensive government corruption and abuse of power. Although Sierra Leone is rich in natural resources it became one of the poorest countries in the world because of mismanagement. The civil war in neighboring Liberia helped to create conditions for war in Sierra Leone because a Liberian war leader reportedly sponsored the rebel group called the Revolutionary United Front (RUF) as a way of destabilizing Sierra Leone, which at the time was a base for a United Nations peacekeeping force. The war broke out in 1991 in villages in eastern Sierra Leone that were near the Liberian border. The aim of the RUF was to seize and control the diamond sector, and in 1991 it took control of the diamond mines in the Kono district. (It is the mining area around Ishmael’s home town of Mogbwemo that the rebels seize in 1993.) In 1992 a military coup took place that established the National Provisional Ruling Council, replacing the civilian government. However, the new military government was powerless to prevent the RUF from controlling much of the country. It was the years immediately after this, from 1993 to January 1996, that Ishmael was a soldier. The war continued after Ishmael was rescued from it, as he himself found out when he went to stay with his uncle in Freetown after his rehabilitation. There had been an election in April 1996, and a civilian government had taken power, but in May 1997 there was another military coup, and the new military government known as the Armed Forces Revolutionary Council (AFRC) invited the RUF to participate in it. The following year, when Beah was safely in the United States, the military government was ousted and the civilian government restored. But this did not stop the violence as the AFRC and its RUF allies fought to regain power. Fighting returned to Freetown in 1999, before a peace accord was signed in July 1999. But this did not last, and the war dragged on, finally ending in January 2002, with the civilian government in charge. According to the CIA’s World Factbook, the civil war resulted in tens of thousands of deaths and the displacement of more than two million people—about one-third of the population of Sierra Leone. 3.What is the situation in Sierra Leone today? According to the CIA’s World Factbook, Sierra Leone is gradually returning to a fully democratic government following the ravages of the civil war. There was a general election in 2007 that led to one civilian government being peacefully replaced by another. The nation has also tried to come to terms with the recent past. In 2002 the government set up a Special Court to try those responsible for war crimes during the civil war. It also set up a Truth and Reconciliation Commission. The Special Court indicted many of those held responsible for the atrocities. Some died before they could be tried, but in June 2007, the Special Court found three men guilty of war crimes, including not only murder, terrorism, and enslavement but also the act of conscripting or enlisting children under fifteen into the armed forces. As refugees from the war are slowly returning from neighboring countries, the Sierra Leone government is trying to create jobs and end political corruption. Revenues from diamond mining have increased significantly since the end of the war. Diamonds account for about half of Sierra Leone’s exports. However, Sierra Leone, with a population estimated in 2009 as 5,132,138, remains an extremely poor country with wide disparities in how wealth is distributed. According to the World Factbook, â€Å"The fate of the economy depends upon the maintenance of domestic peace and the continued receipt of substantial aid from abroad.† 4.How widespread is the use of child soldiers? It would be comforting to think that the forced conscription of children into the armed forces during the war in Sierra Leone was an aberration, not something that can happen again in the modern world. However, that is not the case. Even in the twenty-first century, the use of child soldiers is common in armed conflicts around the world. According to Human Rights watch, an international nongovernmental organization, as of 2007, there were an estimated 200,000 to 300,000 children fighting in various wars. According to a Global Report published in 2008 by the Coalition to Stop the Use of Child Soldiers, at the end of 2007 children were used as soldiers in seventeen armed conflicts around the globe. The coalition noted that this was down from twenty-seven conflicts in 2004, but the downturn was more because the conflicts had ended than because child soldiers were no longer being recruited. The Global Report identified the following countries where children were recruited for paramilitaries, militias, civilian defense forces or armed groups linked to or supported by governments: Chad, Colombia, Democratic Republic of the Congo, India, Iran, Ivory Coast, Libya, Myanmar, Peru, Philippines, Sri Lanka, Sudan, and Uganda. The most flagrant offender, according to the Global Report, is Myanmar, where the government uses thousands of children in its battle against rebel groups. In Uganda, tens of thousands of children have been forced into joining armies over a period of nearly twenty-five years. In some of these countries, including Uganda, girls as well as boys have been forced to become soldiers. There have in recent years been concerted international efforts to end the use of child soldiers. Sierra Leone, which has tried and convicted men responsible for recruiting child soldiers, has become a leader in this issue. The use of child soldiers has now been prohibited by international law. The Optional Protocol to the Convention on the Rights of the Child on the involvement of children in armed conflict has been ratified by 120 states. The UN Security Council has adopted resolutions calling for the establishment of a monitoring mechanism on children and armed conflict. However, these and other prohibitions are no guarantee that when new conflicts break out, child soldiers will not be recruited. 5.Is Beah’s story factually accurate? A Long Way Gone achieved popular and critical success, but questions have been raised by some regarding the factual accuracy of a number of events Beah recounts in the book. Beah writes that his village was attacked in January 1993 and after that he became a refugee from the war. Critics claim that there are school records showing that Beah was in school later than this date, and that the village was attacked in 1995, not 1993. This would mean that Beah would actually have been recruited at the age of fifteen, not thirteen as he writes in the book. This would have meant that he was only a child soldier for a few months, rather than over two years. Some critics point to the structure of the book to confirm this. They point out that most of the book deals with Beah’s wanderings as a refugee and the months he spent in rehabilitation. Only two chapters (13 and 14) cover his actual experiences as a soldier (although he does present more incidents from his military service at various points in flashbacks). Questions have also been raised about the account Beah gives of the fight between the former boy soldiers at the rehabilitation home, in which several boys were killed. There are no independent reports of such a fight ever taking place. Some believe that Beah used others’ experiences as his own and that he embellished his tale. They point to his interest in creative writing at Oberlin College and the fact that his adoptive mother was a storyteller. The suggestion is that Beah was encouraged by those around him to tell a more vivid story. Others have more charitably suggested that Beah simply got his dates mixed up, and his memory may have been unreliable because on his own admission he was high on drugs most of the time he was in military service. Beah has vehemently denied that he invented anything, however. In an article published in Publishers Weekly in 2008, Beah wrote, â€Å"Sad to say, my story is all true.†

Rating Scales One method of appraisal is rating scales, and this is one of the more common forms of performance appraisal found within organisations (Jafari et al., 2009). Two of these which are commonly used are the graphic rating scale as well as the behavioural rating scale. Using the graphical rating scale, the employees are assigned a score based upon different criteria which are important to job success (Kokemuller, 2012). The behavioural scale focuses on the actual behaviours an employee displays. The benefits of rating scales is the fact that they are easy to use and to understand. Many managers and employees are also familiar with this technique, and they offer a simple way to communicate areas of strengths and weaknesses to the employee (Kokemuller, 2012). The graphical rating scale is not very time consuming at all, and they are easy to develop and administer as well as allowing for a range of quantitative analysis. Negatives of rating scales is the assumption that they are tied to a raise or a bonus for the employee, and this can contribute towards score inflation. Managers can also have a negative bias towards employees when utilising this method too (Kokemuller, 2012). Narrative Techniques Another technique is the narrative technique, which is similar to the critical independent method. When compared to the rating scales technique, these offer a more detailed analysis of job performance (Kokemuller, 2012). In these, an essay assessment is written of the performance for the narrative technique, with a log kept for the critical independent method. This log is based on the critical incidents which occur over the year, or the period of time the performance appraisal focuses on, and looks to the actual performance as opposed to behaviours like the essay technique (Jafari et al., 2009). A benefit of these techniques is the amount of detail, and how thorough they can be in analysing the behaviour of employees (Kokemuller, 2012). Maintaining a detailed set of notes throughout the year enables a manager to give a well-rounded and an accurate account of employee performance (Root, 2011). As well as this, there can be a focus on praising positive behaviours, as well as addressi ng areas where correction is needed. However, this thoroughness and scope for wide ranging discussion does leave the analysis up for open interpretation without a scoring and rating system, and therefore the employee may interpret the evaluation in a too positive or too negative light (Kokemuller, 2012). This method can be used in conjunction with other methods, such as the graphical scale, to enable something more quantitative with a qualitative rating too (Jafari et al., 2009). Comparison Methods Comparison methods is a multi-person method, which is a common appraisal technique. This approach, and the forced distribution method, compare the employees performance to the fellow workers within the organisation (Kokemuller, 2012). This allows the manager to communicate to an employee where they are performing well, and where they are not performing well in comparison to the other employees. The benefits of this are that this can motivate performance in competitive workplaces and among employees who have a competitive nature (Kokemuller, 2012). However this comparative methods could cause infighting, lack of teamwork and a lack of harmony among employees within the business, and this could lead to a deterioration in performance (Kokemuller, 2012). This method could be used in conjunction with the graphical scale, as the graphical scale would allow easy comparison between employees (Root, 2010). 360 Feedback The development of an employee is dependent on the progress made within the specific department that they work in, as well as how effective they are when working with the rest of the company (Root, 2010). 360 degree feedback is a popular performance appraisal technique in modern organisations. In this method, the employee is evaluated by a wide range of people. This can range from co-workers, to supervisors as well as customers and those working under them (Kokemuller, 2012). One advantage of this is that the employee can see how a wide range of stakeholders view their performance in the key relationships that are critical to the employees job role. Different perspectives are also a positive in the appraisal process, and this is much more comprehensive than having just one person assessing the employees performance (Kokemuller, 2012). Hakala (2012) notes that 360 degree feedback is the most comprehensive, but also the most expensive performance appraisal measure and is normally onl y reserved for key employees within the organisation. It is able to measure a range of different objectives effectively that other aspects may not, such as interpersonal skills and customer satisfaction (HRwale, 2015). There is also the 720 degree method, which Shaout and Yousif (2014) cite that it is 360 degree but practiced twice. It includes getting feedback from further external sources such as suppliers, family and communities of which the organisation is intertwined with. 360 degree feedback is expensive and time consuming for organisations to implement, as it is much more comprehensive, and requires much more time invested in it to implement as opposed to the other methods previously discussed. Self Evaluation and Team Evaluations Another form of effective performance appraisal has been seen to be self-evaluation and team evaluation. The benefits of these are that they are easy to administer, involving forms that can be filled in and used to rate yourself or those in your team based on a number of different criteria (Root, 2011). These forms can also be used in conjunction with other methods in an attempt to see if their colleagues and self-performance evaluations are of a similar view to the managers. Indeed, Root (2010) mentions that this is particularly useful when teamed with a performance review. Fellow workers may possibly have a better perspective of the jobs that are performed by their peers, and also have a better insight into what working them is liked (Root, 2012). However, there is obviously the possibility that a bias may exist when self-evaluation, or evaluating colleagues who may be friends, or who may be disliked despite having stellar performance in their job roles. This evaluation by peers or by the team can help to open up conversation between the manager, the employee, and the team which can be extremely beneficial to the overall development for the employee (Root, 2010). This is because there can often be discrepancies between what the management, and what the employee consider to be important performance factors (Hakala, 2008). Management By Objectives The final technique of performance appraisal is management by objectives. Here, employees are assessed on how they can accomplish a specific set of objectives that the company have asserted are critical for the successful completion of the organisations corporate objectives (Jafari et al., 2009). Their performance can be graded against the objectives which are specified by the management (Shaout and Yousif, M, 2014). This is a process which converts objectives or organisations to objectives for the individual employees. This requires: setting goals, periodic reviews, plans of action as well as self-control (Jafari et al., 2009). So, the process would begin with statements of action such as reduce defected parts in the manufacturing process to 5%, throughout the process there would be a close monitoring and reviewing of the objectives previously identified to keep the employee focused on their goals (Hakala, 2008). Therefore at the end of year performance appraisal, the progress the employee has made towards their goals is assessed, with new goals set for the next year. This process can be seen to be more useful for managerial positions, as their key goals may be more closely aligned with corporate strategy, and thus easier to identify. Likewise, the negatives may not be applicable to all jobs. Another negative would be the fact that focusing on specific objectives in the performance review may lead the employee to neglect any objectives which are not so heavily scrutinised, or not tied to the allocation of merit pay (HRwale, 2010). Conclusion Is there one best method of performance appraisals? As has been seen in this essay, and as academics assert, there is no one size fits all approach to employee performance appraisal (Scott and Einstein, 2001). It can be seen that a range of different performance appraisal processes are effective when implemented (Root, 2010). As well as this, more and more companies are beginning to recognise that effective performance appraisal is key to successfully achieving corporate objectives (Grote, 2000). Human resource management and performance appraisals are aimed to improve corporate performance, while targeting individuals on the individual level as opposed to the business level. It is important to develop realistic and clear performance standards while reducing communication problems between the managers and their employees. Companies need to select the correct method for them based on a range of different factors, such as the need to compare employees, the cost of the method, the scope for error as well as the training needs evaluatio n for their specific company (Jafari et al., 2009). If performance appraisals are not done correctly, a mixed message can be left which leaves employees feeling disappointed and confused, whereas the purpose of performance appraisals are to reinforce the good work the best performers do, and to try and improve what poor performers do (Knight, 2011). In order for the performance appraisal process to be successful, it needs to have the same quality of businesses who thrive. It needs passion, agility, speed and alignment (Bersin, 2013). Organisations need to be flexible in their processes, and arrange them to meet the targets for their specific company. Organisations also need to be aware of overcoming the negatives of performance evaluation such as having a one sided conversation, or an evaluator bias (Johnson, 2011). If these are present, they can have a negative effect upon employees, which is not intended. Therefore it is impossible to recommend one process over all of the other s. As has been seen, all the processes have negatives and positives and are more applicable in different situations. 360 degree feedback is the most comprehensive of all the methods, but this is expensive and time consuming and can still fall foul of the negative aspects of performance appraisal. There have been many controversies in the history of the United States, ranging from abortion to gun control; however, capital punishment has been one of the most hotly contested issues in recent decades. Capital Punishment is the execution of a criminal pursuant to a sentence of death imposed by a competent court. It is not intended to inflict any physical pain or any torture; it is only another form of punishment. This form of punishment is irrevocable because it removes those punished from society permanently, instead of temporarily imprisoning them, this is the best and most effective way to deal with criminals. The usual alternative to the death penalty is life-long imprisonment. Capital punishment is a method of retributive punishment as old as civilization itself. The death penalty has been imposed throughout history for many crimes, ranging from blasphemy and treason to petty theft and murder. Many ancient societies accepted the idea that certain crimes deserved capital punishment. Ancient Roman and Mosaic Law endorsed the notion of retaliation; they believed in the rule of an eye for an eye. Similarly, the ancient Egyptians, Assyrians, and Greeks all executed citizens for a variety of crimes. The most famous people who were executed were Socrates (Saunders 462) and Jesus. Only in England, during the reigns of King Canute (1016-1035; Hoyt 151) and William the Conqueror (1066-1087; Miller 259) was the death penalty not used, although the results of interrogation and torture were often fatal. Later, Britain reinstated the death penalty and brought it to its American colonies. Although the death penalty was widely accepted throughout the early United States, not everyone approved of it. In the late-eighteenth century, opposition to the death penalty gathered enough strength to lead to important restrictions on the use of the death penalty in several northern states, while in the United States, Michigan, Wisconsin, and Rhode Island abandoned the practice altogether. In 1794, Pennsylvania adopted a law to distinguish the degrees of murder and only use the death penalty for premeditated first-degree murder. Another reform took place in 1846 in Louisiana. This state abolished the mandatory death penalty and authorized the option of sentencing a capital offender to life imprisonment rather than to death. After the 1830s, public executions ceased to be demonstrated but did not completely stop until after 1936. Throughout history, governments have been extremely inventive in devising ways to execute people. Executions inflicted in the past are now regarded today as ghastly, barbaric, and unthinkable and are forbidden by law almost everywhere. Common historical methods of execution included: stoning, crucifixion, burning, breaking on the wheel, drawing and quartering, beheading and decapitation, shooting, and hanging. These types of punishments today are banned by the eighth amendment to the constitution (The Constitution, Amendment 8). In the United States, the death penalty is currently implemented in one of five ways: firing squad, hanging, gas chamber, electrocution, and lethal injection. These methods of execution compared to those of the past are not meant for torture, but meant for punishment for the crime. For the past decades, capital punishment has been one of the most hotly contested political issues in America. This debate is a complicated one. Capital punishment is a legal, prac tical, philosophical, social, political, and moral question. The notion of deterrence has been at the very center of the practical debate over the question of capital punishment. Most of us assume that we execute murderers primarily because we believe it will discourage others from becoming murderers.
